ABSTRACT

Objective:To discuss the efficacy and safety of Shenwu Yishenpian on stage 4-5 chronic kidney disease-nondialysis (CKD) with deficiency of spleen and kidney Qi, and the effect on renal interstitial fibrosis (RIF) and microinflammation. Method:One hundred and twenty patients were randomly divided into observation group and control group. A total of 58 patients in control group completed the treatment (including 2 patients falling off or lost). And 58 patients in observation group completed the treatment (including 1 patient was falling off or lost visit, and 1 eliminated). Both groups got comprehensive treatment of western medicine. Patients in control group got simulated medicine of Shenwu Yishenpian, 4 pieces/time, 3 times/day. Patients in observation group got Shenwu Yishenpian, 4 pieces/time, 3 times/day. The treatment lasted for 6 months until the renal replacement therapy, and the 6-month follow-up was recorded. For every month, blood creatinine (SCr) was detected, and glomerular filtration rate (eGFR) were calculated. The 12-month renal replacement (dialysis or kidney transplantation), progress (CKD4 to CKD5) and mitigation (CKD5 to CKD4 or CKD4 to CKD3) were recorded. Before and after treatment, levels of urea nitrogen (BUN), hemoglobin (HB), plasma albumin (ALB), urine protein quantity (24 hUp) and blood uric acid (UA) levels were detected, deficiency of spleen kidney Qi was scored, and transforming growth factor-β1 (TGF-β1), connective tissue growth factor (CTGF), serum Klotho, tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), interleukin-1 (Lkn-1) and interleukin-12 (IL-12) were detected. And the safety was evaluated. Result:At the 3th and 6th after treatment, SCr in two groups increased (P<0.01), while eGFR decreased (P<0.01). Compared with control group, SCr was less than that in control group (P<0.01), whereas eGFR was higher than that in control group (P<0.01). During 12 months of observation, the reduction rate of CKD was 13.79% (8/58), which was higher than 1.72% (1/58) in control group. The progress rate of CKD was 11.43% (4/35), which was lower than 31.58% (12/38) in control group (P<0.05). Levels of BUN, 24 hUp and UA were lower than those in control group (P<0.01), while levels of Hb and ALB were higher than those in control group (P<0.01). Effect in observation group was better than that in control group (Z=2.051, P<0.05). And levels of TGF-β1, CTGF, TNF-α, IL-6, Lkn-1 and IL-12 were lower than those in control group (P<0.01), and level of Klotho was higher than that in control group (P<0.01). There was no adverse reaction relating to Shenwu Yishen Pian. Conclusion:Shenwu Yishenpian can delay the progress of renal function and CKD, reverse the progress of renal function in some patients, reduce the risk factors of disease progress, reduce the state of micro inflammation and resist RIF, and protect or improve renal function. Its clinical effect is better than placebo, and it is safe to use.

ABSTRACT

OBJECTIVE@#To develop an improved version of the Quality-of-Life Assessment instrument for Lung Cancer Patients Based on Traditional Chinese Medicine (QLASTCM-Lu) and to evaluate its psychometric property.@*METHODS@#The structured group method and the theory in developing rating scale were employed to revise the preliminary scale. The psychometric property (reliability, validity, and responsiveness) of the established QLASTCM-Lu (modified) were evaluated by quality of life data measured in 100 lung cancer patients. Statistical analyses were made accordingly by way of correlation analysis, factor analysis and paired t-test.@*RESULTS@#The internal consistency reliability of the overall scale and all domains was from 0.80 to 0.94. Correlation and factor analyses demonstrated that the scale was good in construct validity. The criterion validity was formed with European Organization for Research and Treatment of Cancer-Quality of Life Questionnaire-Lung Cancer (EORTC QLQ-LC43) as the criterion. Statistically significant changes were found apart from such domain as "mental condition" and "social function", with the standardized response means being close to those of QLQ-LC43.@*CONCLUSION@#QLASTCM-Lu (modified) could be used to measure the quality of life of lung cancer patients with good reliability, validity and a certain degree of responsiveness.

ABSTRACT

In order to explore the anti-inflammatory activity and active ingredient basis from the leaves of the Belamcanda chinensis and Iris tectorum, we established an HPLC method for simultaneous determination of six anti-inflammatory active ingredient contents in the root of the B. chinensis and I. tectorum as well as their leaves with different dry methods, and the anti-inflammatory effects of the extract were studied by the mouse ear swelling experiment. The HPLC analysis was performed on an Agilent WondaSil© C₁₈-WR(4.6 mm×250 mm,5 μm),with isocratic elution of acetonitrile-0.1% ortho-phosphoric acid solution at a flow rate of 1. 0 mL·min⁻¹ and the detection was carried out at 265 nm. The chemical compositions of the B. chinensis and I. tectorum are similar but the contents of them are obviously different. Both rhizome and leaf extract of B. chinensis and I. tectorum had inhibitory effects on inflamed mice induced by dimethylbenzene and had anti-inflammatory effects by animal experiment, which could lay the material and active foundation for the development of the non-medicinal parts of the B. chinensis and I. tectorum.

ABSTRACT

The epithelial-mesenchymal transition (EMT) effect, cell migration, and invasion ability of cancer cells play important roles in the process of cancer metastasis. Resveratrol is a natural polyphenol found in grapes, berries, and peanut skins. Many experimental studies have shown that resveratrol and its metabolites and analogs have multiple targeting anti-cancer effects. The aim of this study is to provide resveratrol and its metabolites and analogues to inhibit the potential for cancer metastasis by influencing many signaling pathways associated with EMT, cancer migration and different organ invasion. We found that resveratrol’s metabolites and analogues can inhibit the EMT process by affecting the role of multiple signaling pathways, inhibiting the metastasis and erosion of cancer through the literature review.

ABSTRACT

<p><b>OBJECTIVE</b>This study was to explore the correlations between genetic variants in MMP2-1306C/T, TIMP2 2379C/T, TIMP2 303G/A and the genetic susceptibility to gastric carcinoma (GC) in Fujian province, China.</p><p><b>METHODS</b>A case-control study was conducted. Polymorphisms of MMP2-1306C/T, TIMP2 2379C/T, TIMP2 303G/A in 479 gastric carcinoma patients and 469 cancer-free controls, frequency-matched by age and sex, were determined by Matrix-Assisted Laser Desorption/Ionization Time of Flight Mass Spectrometry (MALDI-TOF-MS). Multivariate logistic regression analysis was used to evaluate the correlations between the polymorphisms with the susceptibility to gastric cancer. Tests for an interaction between the MMP2-1306C/T and TIMP2 2379C/T or TIMP2 303G/A were performed using the likelihood ratio test.</p><p><b>RESULTS</b>The frequencies of GG, AG, AA of TIMP2 303G/A in gastric cancer group were 55.9% (267/478), 38.7% (185/478) and 5.4% (26/478); and those in control group were 53.1% (243/458), 36.9% (169/458) and 10.0% (46/458), which showed a significant difference between the patient and control groups (χ(2) = 7.0, P = 0.03). As compared with AA genotype, patients with GG + AG had a significantly higher risk of the cancer with OR of 1.94 (95%CI: 1.2 - 3.2). The frequencies of GG + AG and AA of TIMP2 303G/A in the muscle group were 87.5% (70/80), 12.5% (10/80), however, those in the serosa and beyond group were 96.0% (382/398), 4.0% (16/398), respectively, which showed a significant difference between the patient in muscle group and the serosa and beyond group (χ(2) = 9.32, P = 0.002). As compared with AA genotype, patients with GG + AG had a significantly higher risk in tumor invasion with OR of 3.4 (95%CI: 1.5 - 7.8). The frequencies of CC + CT, TT of TIMP2 2379C/T in the lymphoma node non-metastasis group were 93.4% (113/121), and 6.6% (8/121), but those in the lymphoma node metastasis group were 98.6% (349/354) and 1.4% (5/354), respectively, which showed a significant difference between the patient in the lymphoma node non-metastasis group and in the lymphoma node metastasis group in genotype distributions of TIMP2 2379C/T) χ(2) = 9.16, P = 0.002). As compared with TT genotype, patients with CC + CT had a significantly higher risk in lymph node metastasis with OR of 4.9 (95%CI: 1.6 - 15.3). MMP2-1306C/T genotype was not associated with tumor size, tissue differentiation, invasion, TNM nor lymph node metastasis of gastric carcinoma (χ(2)(tumor size) = 0.05, P = 0.98; χ(2)(depth of invasion) = 1.87, P = 0.39; χ(2)(histological type) = 0.55, P = 0.76; χ(2)(LN metastasis) = 0.44, P = 0.80; χ(2)(TNM) = 2.6, P = 0.28). There was no significant interaction between the polymorphisms of the two genes observed (χ(2) values were 0.98 and 0.80, P values were 0.81 and 0.85).</p><p><b>CONCLUSION</b>Polymorphism of TIMP2 is significantly related with occurrence and development of GC and maybe acts as a new biomarker of GC in prognosis.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the effect on erythropoietin (Epo) resistance between acupoint injection and subcutaneous injection of rHuEpo in patients with chronic renal failure (CRF).</p><p><b>METHODS</b>Thirty-eight cases were randomly divided into two groups, 19 cases in each one. In subcutaneous injection group (control group), subcutaneous injection of rHuEpo was administered, 3 times a week, lasting 2 months. In acupoint group (observation group), rHuEpo was injected on unilateral Shenshu (BL 23) and Zusanli (ST 36), one point was chosen each time, the bilateral acupoints were injected alternatively, 3 times a week, for 2 months. Meanwhile, a normal control group of 19 healthy persons was set up. The levels of CRP, IL-6, TNF-alpha, Scr, BUN, Hb, Hct and SF were observed.</p><p><b>RESULTS</b>Before treatment, the values of CRP, IL-6 and TNF-alpha in two groups were all higher than those in normal control group (all P < 0.01). After treatment for 2 months, the values of CRP, IL-6,TNF-alpha, Scr and BUN in two groups decreased apparently and those of Hb, Hct and SF increased obviously, indicating statistic significant differences as compared with the values before treatment separately (P < 0.05, P < 0.01). In comparison between two groups after treatment, every index above in observation group was improved much significantly (P < 0.05, P < 0.01).</p><p><b>CONCLUSION</b>Acupoint injection of rHuEpo at Zusanli (ST 36) and Shenshu (BL 23) increases significantly the values of Hb, Hct and SF, and decreases apparently the values of BUN, Scr and inflammatory factors, such as CRP, IL-6 and TNF-alpha as compared with subcutaneous injection. Acupoint injection improves Epo resistance and enhances Epo efficacy via alleviating micro-inflammatory state of the body.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the correlation of functional genetic variants in Nme1-509 C>T and TGFβ1-1465 T>C genes to the genetic susceptibility of gastric carcinoma in Fujian province, China.</p><p><b>METHODS</b>A case-control study was conducted in a population in Fujian province. The polymorphism of TGFβ1-509 C>T (rs1800469), Nme1-1465 T>C (rs16949649) in 273 gastric carcinoma patients and 277 cancer-free controls, frequency-matched by age and sex, were analysed by using Matrix-Assisted Laser Desorption/Ionization Time of Flight Mass Spectrometry (MALDI-TOF-MS). Adjusted odds ratios (OR) and 95% confidence evaluation intervals (95%CI) measured by multivariate Logistic regression analysis were adopted in studying the correlation of the gene polymorphism with the susceptibility of gastric cancer.</p><p><b>RESULTS</b>After the adjustment using Logistic regression or the potential confounding effects of gender and age, as compared with TT+CT genotype gastric carcinoma patients, the homozygous Nme1-1465CC genotype carriers had a significantly higher risk in lymph node metastasis, with the OR of 2.5 (95%CI 0.08-2.10; P=0.029). There was no association obtained between TGFβ1-509 T>C genotype with the tumor size, cell differentiation, tumor invasion and lymph node metastasis in gastric carcinoma. In the intestinal type gastric carcinoma group, when compared with the wild homozygous Nme1 TT* TGFβ1 CC, Nme1 TC* TGFβ1 TC, Nme1 TC* TGFβ1 TT and Nme1 CC* TGFβ1 TC genotype carriers, there was a significantly decrease of risk in gastric carcinogenesis of 0.42 fold (95%CI 0.54-0.94, P=0.022), 0.32 fold (95%CI 0.42-0.97, P=0.013) and 0.26 fold (95%CI 0.42-0.97, P=0.008), respectively.</p><p><b>CONCLUSIONS</b>There is a significant relationship between polymorphism of Nme1-1465 T>C and the prognosis of carcinoma of stomach. It also demonstrates that coexistence of Nme1-1465 T>C and TGFβ1-509 T>C genes may provide a synergistic effect of increasing the susceptibility of gastric carcinogenesis.</p>

ABSTRACT

<p><b>BACKGROUND</b>To find out the timing of serologic responses after illness onset and distribution of IgG antibody to SARS-CoV in SARS cases of transmission chain or non-transmission chain.</p><p><b>METHODS</b>The IgG and IgM antibodies to SARS-CoV were tested by indirect ELISA in serum samples from 301 clinically diagnosed SARS cases.</p><p><b>RESULTS</b>Totally 158 SARS cases were involved in 15 chains of transmission. The positive rates of SARS-CoV IgG in those chains were 85.70%-100.00% and the overall rate was 94.30% (149/158). The chain of transmission could spread to four generations, but the SARS cases were reduced with increase of generations. There was no significant difference among positive rates of SARS-CoV IgG for generations, Chi square=5.11, P greater than 0.05. The positive rate of SARS-CoV IgG in cases who were not in chain of transmission was 12.59%(18/143) which was statistically significantly different from that of cases in chain of transmission, Chi square=199.64, P less than 0.001. During days 0-7,8-14,15-21,22-30 after onset, the cumulated positive rate of SARS-CoV IgG was 16.67%, 40.00%, 70.00% and 93.10%, respectively, then was kept at the level above 90% and lasted for 217 days. The cumulated positive rate of SARS-CoV IgM during days 0-7 after onset was the same to that of IgG. During days 8-14, 55.17% of cases had seroconversion for IgM which reached a peak (86.96%) during days 21-30. Then the rate rapidly declined.</p><p><b>CONCLUSION</b>More than 94% of cases with SARS could produce IgG antibody when they were infected by SARS-CoV. Detecting SARS-CoV IgG could provide a diagnostic evidence for case confirmation. SARS-CoV IgG appeared as early as 7 days after onset and reached the peak at about weeks 4. Then the high rate of antibody was maintained for more than 6 months.</p>
